Using the data from your PLC Assessment instrument, devise a plan that you would present to faculty or colleagues in an effort to promote buy-in in order to address the problem identified in the assessment findings. 
Your plan should be approximately 4 pages and the format can be an expanded outline rather than straight narrative.  In other words, you can use bulleted information rather than paragraph format.  The crucial part in this report is the details you provide.
Your plan can NOT be a one-shot plan (something you do one time on one day like at a faculty meeting).  The plan needs to be sustainable over time such as a semester or 9 weeks or school year, etc. 
 Here are the categories to include in your plan and some ideas for what to include in your plan within each category.  Also, look at the scoring rubric for more help.
1.  Group(s).  Include who, how selected, and why.  Provide details about these members. Give a very clear picture of the makeup of the groups. 
2. Schedule.  Make your schedule of when these groups meet very detailed.  Include information related to how your school schedule has been taken into consideration.  Make this part about the time frame easy to read and follow. 
3.  Needs Assessment and Rationale.  You need to have a clear and detailed explanation of how the goal(s) of the project were determined.  How you used data should be included.  Information regarding the knowledge and skills needs of the teachers should be in this section. 
4.  Budget and Resources.  You cannot start professional development without a budget and knowledge of resources available.  Provide a detailed budget.  Include specific names and contact information regarding consultants or other resource personnel that you may be using in your plan.
 5. Professional Development Plan to Meet the Goals and Objectives.  This part should provide extensive and detailed professional development relating to instructional strategies aligned to the goals and objectives. In other words, this is what you are going to do, how this is aligned to your goals and objectives and how will you know you met the goals.
6.  Dysfunctions.  Identify two of the five dysfunctions your team is most likely to be susceptible.  In detail, describe your plan to avoid these pitfalls.
