Program evaluations are systematic studies conducted periodically or on an ad hoc basis to assess program performance. Evaluations help leaders determine if timely adjustments are needed in program design to improve the quality of services/products. Your Program Evaluation Paper will be a PLAN for an evaluation.  You will not be required to actually complete a program evaluation for this course. Pretend you’ve been instructed by your CEO to conduct a program evaluation. You are writing the document that you will give to him/her for approval before you actually start the evaluation. Show your CEO that you have a good plan for your evaluation so that he/she will instantly approve!
For Part 1 of your Program Evaluation Paper, please describe, in detail, 1) Rural health clinic and 2) the program within the organization that you wish to evaluate (School Based Clinic). For instance, is there a current fall program or a current rounding process at your place of employment that needs to be evaluated?  Look for programs/projects at your organization that likely need some updating. Information regarding different types of programs can be found on pages 31-33 in your text (Program Evaluation forms and approaches). Reading that section may provide you with some ideas of a program that can be used for your evaluation plan. 
For Section 2 of your Program Evaluation Paper, determine which evaluation FORM you will use for your program evaluation, describe it, and provide a justification for choosing/using that form. There are five evaluation forms: proactive, clarificative, interactive, monitoring, and impact.  Information on the different forms can be found in Chapter 3 in your text.  Tables 3.1 and 3.2 (pages 52-53) and Table 3.3 (page 61) may be particularly helpful in determining which form is best for you to use. There are also dedicated chapters for each form in your text (Chapter 9: Proactive, Chapter 10: Clarificative, Chapter 11: Interactive, Chapter 12: Monitoring, and Chapter 13: Impact) which may be helpful. This will probably be the most difficult part of your paper–let us know if you need our help! 
Your ENTIRE Program Evaluation Paper will need to be at least 5 pages and in APA format.  Please be sure to review the grading rubric for the paper.  Submit Part 1 of your paper here.  We will provide feedback on each submission and you can use the feedback to make improvements with each submission.
