Overview:
You have completed a community assessment and analyzed and diagnosed community concerns and/or conditions. You have planned a health promotion intervention and developed the material necessary for implementation for the identified community/aggregate; finally, you have developed a method for evaluating the outcomes of the health promotion intervention and you pulled it all together in a cohesive document. Dissemination of information is the final step in your Health Promotion initiative. The way that you organize and communicate your ideas is key in articulating the element of your plan in a succinct, descriptive and cohesive manner.
This week, you will present your health promotion project to your “stakeholders” (in this case your classmates) to stimulate thought and spur ideas. You have a choice of two formats; a poster or a power point presentation.
Objective:
For this task, you will:
Prepare a Poster OR Presentation of your Health Promotion Project for class review, submit it for grading AND post it to the Final Presentation Discussion Forum.
IMPORTANT: Before you build your presentation or poster, ensure you have implemented any instructor feedback throughout the course. Repeat errors in your presentation, which were identified earlier through feedback, will result in increased loss of points. Use your compiled paper (submitted in Week 7) as framework for your poster or presentation. You will see that the progression of the elements below closely follows your paper. A printable copy of this page is at the bottom so that you can follow the steps closely as you create your final poster/presentation.
At each stage of the nursing process in this course, you have been creating materials which will now be collected into one presentation which will be submitted for grading and posted to the Course Discussion Board for review by your classmates.
Ensure that instructor guidance/feedback from previous weeks is incorporated into your final presentation.
The Poster or Presentation must include Each ONE of the elements below; in a Power Point they should be addressed in the following order:
Assessment
Identification of the community assessed
Brief list of most compelling data
Identification of social determinants which impact the community
Identification of Community Health Diagnosis/Problem including cited evidence and conditions related to the problem.

Include samples of your materials to support your implementation-if you are creating a poster and cannot fit them all, ensure you describe them and then submit the materials as an appendix.
Identify the Level of Prevention your health promotion intervention provides.
Evaluation
Identify methods of evaluation and provide the what, how, who, and when of each.
Identify any health literacy concerns.
Identify any adjustments needed to ensure cultural safety.
Identify potential ethical issues.
Identify at least one health policy connected with the Health Promotion intervention.
References-
If you are presenting a Power Point, you should include all data sources, library and outside references used in your process paper as you used these sources to create your presentation as well. Use a font large enough that you can view it easily on the slides (you can check this by going to slide show view). The NURS 362 course resources you have used throughout the creation of your health promotion may be submitted as an appendix. You may have multiple reference slides. References do not need a hanging indent or double spacing, however, they need to be styled properly and there should be double spacing between references.

Appendices will include additional implemetation and evaluation materials not included in the presetnation or poster and NURS 362 Course Reference in APA style.
You can create either a Poster or a PowerPoint presention as long as it, or a link to it can be submitted for grading and uploaded onto the course discussion board.
Power Point
Speaker’s notes MUST be included to provide context to slides.
Please note: The slides themselves need to include the pertinent information. In other words, they should not simply be an outline of the content you will be addressing, even if you address the elements in your speaker’s notes. Each slide must contain the actual facts/points you are trying to make in the presentation.
PowerPoints must include citations when appropriate, either on the slide or in the speaker’s notes. Remember to cite ALL of your data!
If creating a PowerPoint use the 6 X 6 rule as a guide. Major ideas/components should have their own slide. It is always better to have multiple slides than too many items or words on one slide. This applies to the entire presentation.
Poster
If you choose to create a poster, each of the required elements must be included and be clear and self explanatory.
Include images, charts, photos etc. Ensure that they are cited if they are from source materials. Use a combination of paragraphs, bullets, images, charts, graphics, etc. on your poster.
You may use any platform you wish to create the poster. It should be large enough to accomodate all of components. The sample template below is 56 X 31 and was created with Power Point. Citations should be used in your content when applicable.

If you create something your classmates and instructor must link to, then it is your responsibility to make sure they will be able to access it, i.e., you will need to make the piece accessible or share it appropriately.
Submit your presentation/poster within the assignment AND,
Post your presentation to the Course Discussion Board: Health Promotion Presentations.
When you post to the Health Promotion Presentation forum, please put the name of your city/community and the topic of your health promotion intervention, e.g., Sterling, IL – Dental health for children; Fuller Park/Chicago – Vaccine access.
